Senior Day Services | Meals on Wheels | Congregate Dining | Rehab Services | Child Day-Care

SENIOR DAY SERVICES

Senior Day Services is a community-based program offered through Cokato Manor to provide services and activities enabling functionally impaired individuals to remain in their homes and a part of the community longer. This service also provides respite relief to their caregivers.

Senior Day Services include an opportunity for mental stimulation, creative expression, spiritual growth and personal enrichment through a large variety of activities.

The activities are a great opportunity for social interaction and time for new friendships to develop. Waiverd Service and private pay are accepted. Transportation is a 20 mile radius of Cokato. Meals, snacks, daily activities, common fitness, medications, bathing and spiritual development are all included.

Hours: Monday-Friday 8:00am to 4:30pm (excluding holidays).

MEALS ON WHEELS

Meals on Wheels is a program that provides nutritious meals to the residents of Cokato in need of a healthy and nutritious meal delivered to their home 7 days a week.

The meals are low in salt and low in fat and can be altered to meet the needs of therapeutic diets.

Congregate Dining

Congregate dining is offered as a noon meal with friends and neighbors.

Along with the meals, nutrition advice and education is provided to participants on a regular basis. There are a variety of meals served and they can be modified to fit any nutritional need.

The meals are served Monday through Friday at 12:00pm in the Cokato Apartments I Community Room.

Rehab Services

Health and Housing in partnership with Health Demention Rehab, Inc.

Providing comprehensive rehabilitation services which include:

  • Physical therapy
  • Occupational therapy
  • Speech/Language therapy
  • Orthotics and Prosthetics

Rehabilitation provides each client with the tools needed to maximize individual potential for achieving personal goals. We work with people of all ages who, because of physical, developmental, social or emotional problems, need specialized assistance to lead independent, productive and satisfying lives:

  • Physical therapy focuses on restoration of normal movement, relief of pain, strengthening and education for self management
  • Occupational therapy focuses on restoring and teaching the skills for the job of living.
  • Speech/Language therapy focuses on communication impairments, voice, swallowing disorders and hearing loss adaptions.
  • Orthotists are trained to access, fabricate and fit clients for external supports to correct alignment and improve the mechanics of joints.
  • Prosthetists are trained to assess, fabricate and fit clients for artificial limbs.

ALL SERVICES REQUIRE A DOCTOR'S ORDER AND CAN BE PROVIDED ON AN IN-PATIENT, OUT-PATIENT OR HOME HEALTH BASIS.

The Community Fitness program has expanded to include a wellness program for community involvement. The program uses Nautilus equipment which has been specifically designed to provide comfortable, safe and highly effective strength building. Strength preservation throughout life can prevent or lessen the effects of osteoporosis, arthritis, diabetes and heart disease.

A staff trainer is present to teach correct technique and to help customize an exercise regime to fit each participant's needs. A membership form can be picked up at the Cokato Manor Physical Therapy department during business hours.

Child Day-Care

Cokato Manor's "The PlayHouse" child daycare is a home daycare within a facility.  Included in the program are daily planned activities, snacks, and interaction with residents within our facility.  Your child may participate full or part time depending on your needs.  Staff is trained with child and adult CPR and first aid.  They also receive continued education on a regular basis to enhance their knowledge to work with children.
