Cokato Manor: 56-bed skilled nursing facility.

Heritage Place: 10-bed special care unit for dementia and memory loss.

Edgewood Gables: Independent senior living apartments with services (30 available).

Cokato Manor Home Health: services provided in residents' homes.

Community Services: senior day services, meals on wheels, congregate dining, rehab services, home health care, transportation and child day-care.

Brookridge: 16 unit assisted-living efficiency and one-bedroom apartments.
